Pediatric Vision

Adult Dental and Vision Buy-Up Plans*
Preventive and Diagnostic Dental
- Routine cleanings
- Oral exams
- Bite-wing, full-mouth, and panoramic film X-ray exams
- Topical fluoride
Basic Dental Care
- Minor restorative fillings
- Endodontics
- Scaling and root planing
- Periodontal maintenance
- Simple extractions
- Adjustments, repairs, relines, and rebases for prosthodontics
Major Dental Care
- Crowns and bridges
- Dentures
- Complex extractions and surgical services
Vision
- Annual exam and refraction
- Standard eyeglasses or contact lenses annually
Member coinsurance payments are required for basic and major dental services.
Adult Dental and Vision Buy-Up plans are available for all adults 19 years of age and older in all Ambetter markets except: Washington, New Mexico and New Jersey. Kentucky offers the buy-up for ages 21 and older because pediatric coverage is through age 20. There is a $1,000 maximum benefit on adult dental plans.
