Use Our Helpful Enrollment Checklist
If you don’t know what you need or how to buy health insurance, use our Ambetter Health enrollment checklist! You’ll need to provide the following basic household information when you begin shopping for a healthcare plan:
Social Security Numbers or other ID numbers for anyone in your household who is applying for coverage.
Income information, which may include tax statements, pay stubs or W-2 forms, for anyone in your household who is applying for coverage.
Current health insurance policy numbers for your family members who already have health insurance.
Employer Coverage Tool, which is a government form you’ll need to complete if you currently have the option to get health coverage through your employer or spouse’s employer. You can find this form on healthcare.gov.
