Open Enrollment is from November 1, 2017 – December 15, 2017. When you begin shopping, you’ll need to provide some of your basic household information. Take a look at our checklist below – it’ll help you get prepared to buy health insurance for yourself and your family.

  
Social Security Numbers or other ID Number

Have these available for anyone in your house applying for coverage.
 

  
Income Information

This may include tax statements, pay stubs or W-2 forms. Be sure to have this information for anyone in your house applying for coverage.
 

  
Current Health Insurance Policy Numbers

If there are members of your family that already have health insurance coverage, you'll need to provide these policy numbers.
 

  
Employee Coverage Form

This is a government form that you'll need to fill out if you currently have the option to get coverage through your employer or your spouse's employer.
