Durable Medical Equipment (DME) and Home Health Care (HHC) dedicated fax line

Effective as of June 3, 2024 Ambetter from Sunshine Health is pleased to announce the inpatient discharge only requests for Durable Medical Equipment (DME) and Home Health Care (HHC) dedicated fax line is now operational, and providers can send requests to 1-833-422-1462. We urge all providers to utilize the prior authorization form alongside the dedicated fax line for DME/HHC Inpatient Discharge requests.
