New Prior Authorization Requirement for Certain Procedures: Effective January 1, 2027

Superior HealthPlan, Ambetter from Superior HealthPlan, Ambetter Health, and Wellcare By Superior HealthPlan Dual Align, is adding prior authorization to the following procedure codes for certain Cardiac Procedures effective January 1, 2027. These updates will create a more consistent set of prior authorization requirements across the Texas health plans and programs.

Each of the affected codes in the following table that will require prior authorization will be processed by Evolent Specialty Services, Inc. a Texas licensed URA # Inc., URA #1777359 (Evolent). If you have questions about specific prior authorization requirements or how these changes affect your practice, please reach out to your local Provider Engagement Representative.

Prior authorization requests can be submitted to Evolent through their provider portal at www1.RadMD.com, by calling 1-800-642-7554, or fax to 1-800-784-6864. Superior ensures medical necessity review criteria are current and appropriate for members and the scope of services provided.

The following Cardiology codes will require prior authorization through Evolent Specialty Services beginning January 1, 2027. 

*Prior Authorization does NOT apply to Medicaid (STAR, STAR Health, STAR Kids, and STAR+PLUS) members under age 21 and Ambetter (Ambetter from Superior HealthPlan and Ambetter Health) members under age 18.
