Ambetter from Superior HealthPlan would like to inform providers that CPT Code 0449U will no longer be a covered benefit as of January 1, 2026.

Please note the following code details:

• This is a Proprietary Laboratory Analysis (PLA) code that is only utilized by one provider.
• Similar services are available through in-network labs, such as Quest and LabCorp.
