EPO Plans – EPO plans, or Exclusive Provider Network plans, cover only in-network care, but can often times offer more provider options. Ambetter from Superior HealthPlan’s EPO plans are underwritten by Celtic Insurance Company.

HMO Plans – HMO plans, or Health Maintenance Organization plans, often have lower premiums and deductibles. Typically a member is required to select their Primary Care Provider upon enrollment and requires referrals for specialists. Ambetter from Superior HealthPlan’s HMO plans are underwritten by Superior HealthPlan.
