2022-01: Leg Stent Coding Requirements

Effective April 15, 2022, Ambetter will require prior authorization review for Leg Stent Coding.
Ambetter will require prior authorization for the following codes starting April 15, 2022. Providers can check codes on the prior authorization tool and submit a request through our provider portal.
