Members must complete the applicable wellness activities to qualify for up to one benefit year of program eligibility. Amazon Prime membership is provided on a month-to-month basis, subject to ongoing eligibility, active coverage, program availability, and Amazon's applicable terms and conditions. To receive the benefit in future years, members must again satisfy all applicable eligibility and wellness activity requirements for the benefit year, if the program remains available. Limited to one Amazon Prime membership per household, regardless of the number of covered members or dependents enrolled under the policy. Dependents are not eligible to receive a separate Amazon Prime membership. Subscribers must maintain active and eligible Ambetter Health coverage. Access to the program may end immediately upon termination of coverage.
