What is a Care Manager?

Care managers are registered nurses or social workers. Your care manager is your partner in coordinating and communicating with your whole care team.

Why Should I Work with a Care Manager?

Your care management benefits can help you stay on top of your health and wellbeing. Your care manager can:

Coordinate

  • Communicate with your doctors, providers, primary care provider (PCP), and caregivers
  • Set up appointments with doctors, providers or your primary care provider
  • Keep track of prescriptions and set up pharmacy home delivery
  • Connect you to the healthcare providers, services, and resources you may need

Educate

  • Explain your health plan benefits 
  • Provide educational resources to understand and manage your health conditions
  • Answer questions about your health, medications, and needs
  • Answer questions about your plan, providers, or resources

Support

  • Create a plan so you can work toward your health goals
  • Offer tips and resources for improving your overall physical and mental health
  • Locate community resources — everything from support groups to food banks
  • Peer support services – Get support for substance use disorder with someone who has been through their own substance abuse recovery journey.
  • Make sure you’re getting the care you need

Caregivers Can Also Use Care Management Services

Caregivers are an important advocate for the needs of members. If you have caregivers or family members who help you manage your health needs, care management services can also support them. Our care managers can work with these approved caregivers to provide support, resources, and assistance and to help the members get the care they need.
