Helping Members Choose Tomorrow: Zero Suicide Prevention Program

Suicide is among the top 10 leading causes of death in the United States, claiming more than 47,000 lives annually. (CDC) The financial impact is also staggering, with suicide and suicide attempts costing the nation over $70 billion per year in medical and work-loss expenses.
At Peach State Health Plan, we recognize the role that social determinants of health (SDOH) play in suicide risk. That’s why we’ve launched Choose Tomorrow™, a Zero Suicide Prevention Program developed by Centene Advanced Behavioral Health. This initiative leverages predictive risk modeling to help identify members at risk of self-harm—before a crisis occurs.
How Providers Can Make a Difference
Suicide prevention starts with early identification, effective intervention, and compassionate care. Through Choose Tomorrow, we aim to:
- Reduce preventable loss of life in families and communities
- Identify suicide risk through data analytics, screening, and assessment
- Provide evidence-based interventions and best practices
- Support provider-driven referrals and care coordination
- Offer comprehensive training to enhance provider confidence in assessing and addressing suicide risk
- Monitor outcomes and continuously improve our approach
How the Program Works
Peach State Health Plan proactively reaches out to at-risk members to conduct suicide risk assessments and develop Suicide Safety Plans (SSP). Care managers connect members with behavioral health providers, crisis resources, and community support services. We also assist with scheduling appointments to ensure continuity of care.
