Ambetter Health Will No Longer Reimburse HCPCS Code G0439 Effective August 31, 2026

Ambetter Health will no longer cover and reimburse services rendered under HCPCS Code G0439, effective August 31, 2026.
Providers should no longer submit this code after August 31, 2026, because they will no longer be reimbursed for it. Please use a more appropriate code when rendering services to your patients.
Code | Description |
G0439 | This code is for a subsequent annual wellness visit (AWV), which includes a personalized prevention plan of service (PPPS) and is a yearly follow-up to the initial AWV. This code is used for follow-up visits occurring 12 months or more after the initial AWV, during which the provider reviews the personalized prevention plan, updates risk assessments, and screens for functional abilities and safety. |
