Recredentialing Process Overview
Delaware First Health initiates recredentialing emails approximately 90 days prior to a practitioner’s credentialing expiration date. This process ensures compliance with State, Federal, and Contract regulations. Recredentialing is required every three years.
Tips for Successful Recredentialing
To avoid delays, please ensure the practitioner’s Council for Affordable Quality Healthcare (CAQH) record is accurate and up to date, including the current contact and credentialing email address. The credentialing team relies on CAQH information for outreach if additional documentation is needed.
Key Requirements:
- Correct contact information in CAQH.
- All required documents (e.g., license, DEA, CLIA, insurance) are uploaded and not expired.
- Admitting privileges details are completed in CAQH.
- An updated W-9 will be requested.
- Disclosure of Ownership will be requested.
Important Notice
Failure to respond to information requests in a timely manner may result in termination. Once termed, the only way to recredential the practitioner is through re-enrollment, which requires completing a new application.
